Team Roster Form

The Tennessee Senior Olympics Board of Directors realizes that circumstances may arise (especially medical) to prevent an athlete from attending a district event.  Many of our sports require district participation in order to advance to the state finals.  If this was an impossibility, please review the hardship form below as all district qualifying opportunities for the 2026 state finals have taken place.  Click below for a hardship form and procedures.
